Funding Opportunity to Support Collaborative Community Efforts toward Achieving 100% Access in Oregon and Southwest Washington

The Northwest Health Foundation, National Policy Consensus Center, Communities Joined in Action, and the Oregon Office of Health Policy and Research are seeking proposals from community collaboratives in Oregon that want to begin or advance their work on achieving the eight critical health access activities identified at the November 17th Oregon-Washington 100% Access Summit.1 It is anticipated that successful applicants will propose broad community-wide efforts involving many public and private stakeholders that coordinate resources, work, incentives, and capacity.
  • Each grant will cover 15 hours of facilitation time plus the required preparation.
     
  • Four grants will be awarded through this process.

Applicant requirements

  • At least one person participating in the project must have attended the Oregon-Washington 100% Access Summit on November 17th held in Portland.
  •  Applicants must come for Oregon or the following Southwest Washington counties: Clark, Cowlitz, Pacific, Skamania, and Wahkiakum.

Proposal requirements

Please provide a one- to two-page proposal that explains your community collaborative effort and how you plan to use the facilitation time. The proposal must address how the community intends to sustain the collaborative effort beyond the funded facilitation, such as the vision for long-term shared leadership that will improve access to health services and improve health outcomes.  The proposal should also address the anticipated outcomes of the community collaborative. Priority will be given to collaborative efforts that provide a local match, thereby increasing the number of facilitation hours.

The proposal must also include:

  • A timeline of the collaborative effort pertaining to the eight critical activities, which includes the number of facilitation hours requested and, if relevant, the additional hours of facilitation that will be provided by the community collaborative (note that one hour of facilitation, including preparation, follow-up, and travel time, will cost roughly $300), and
     
  • A list of who will be involved in the effort requiring facilitation (names and organizations).
