Description:
Coordinated Care Network (CCN), a founding member of Communities
Joined in Action and a three-year recipient of HRSA's Healthy
Communities Access Program grant, will present the access models they
developed for affordable pharmaceuticals utilizing the 340B drug
discount program.
Members of CCN's management team will detail the specialty pharmacy
and case management services they offer as a contract pharmacy to
qualified 340B covered entities. To date, CCN has serviced more than
37,000 patients and dispensed over 800,000 340B medications.

Description:
This sixth call in the Sustainability Series will assist new
community collaboratives in understanding the need for Outcome Measures
to support their programs sustainability effort. You will hear from new
and experienced Directors from various access model programs, who will
share how they utilized the outcome measure data collected for reporting
purposes and sustainability efforts.
These Directors will share examples of the
types of reports they have developed for various audiences including their
community and local government. The panel will discuss the use of outcome
measure data for quality improvement purposes and results obtained.

Description:
This fifth call in the Sustainability Series will assist new
community collaboratives in understanding the need for information
systems to support their programs sustainability effort. Determining
the types of information systems and software support will be discussed.
The steps to developing a comprehensive RFP (Request for Proposal) as
well as an example proposal will be presented.
Maximizing the capabilities of an information system and supporting
software programs after implementation will be discussed. Sample
assessments and lessons learned in establishing and maintaining
information system connectivity will be shared.

Description: This TA call is an entry-level training for
directors of new community collaboratives on the role of marketing
and educational tactics to support sustainability. The goal of this
second TA call will be to answer the question “What marketing pieces
should I put in place as a regular aspect of my agency operations,
and why are these pieces important to sustainability? “ Participants
will review strategies and tactics to disseminate a solid
understanding of a healthcare collaboratives work and value to the
community through marketing.

is targeted for communities beginning to develop medical assistance
plans or with relatively new programs. This is not intended for
advanced communities. The goal of this fundamental call will be to

define data collection methods for the development of a ROI
