Join Communities Joined in Action, along with your peers and colleagues to learn about new tools, methods for success, and actions steps that you can incorporate into your programs and communities to meet our mission of eliminating health disparities.

BREAKOUT SESSION TRACKS

Track 1: Evaluation and Applied Research – This breakout track is designed to provide a forum to bring together researchers, community leaders, and social practitioners who desire to use data and research methods to improve health in our communities. Sessions in this track will include data collection and analysis, research methodologies, identification and analysis of outcome measures for program and/or policy impact and sustainability.

Track 2: Community Health – This breakout track is designed to provide a forum to bring together program staff, policy makers, community leaders, and social practitioners who desire to improve health in our communities. Sessions in this track will highlight outcomes and “how-to” of implementing programs, policies, and evidence-based tools for community health improvement efforts.

Track 3: Multi-Sector Partnerships – This breakout track is designed to provide a forum to highlight best practices to improve community health by collaborating across sectors and organizations. Sessions in this track will highlight outcomes and the processes undertaken by multi-sector partners to collaboratively provide community and clinical linkages to reduce and eliminate health disparities.
