Health Transformation: CJA members and friends are encouraged to nominate an individual, organization, or community health coalition / collaborative that has demonstrated a breakthrough model that achieves better health outcomes, reduces inequities / disparities and or promotes a culture of health – and is adaptable in other settings.

Grassroots Political Engagement: CJA members and friends are encouraged to nominate leaders with exceptional skills in organizing grassroots participation and or utilizing the policy / political process to advance health improvement and equity work. This award may be given to an individual, agency or a community health coalition / collaborative.

Emerging Leader in Health Transformation: CJA members and friends are encouraged to nominate extraordinary community health leaders, in the field for 10 years or less, 40 years old or younger, and with a demonstrated commitment to health and equity.

Marilyn Hughes Gaston Health Equity: CJA members and friends are encouraged to nominate leaders who have significant influence on theory, policy and or practices that address health disparities and promote equity.

Mary Lou Andersen Lifetime Achievement in Healthcare Transformation: The CJA Board of Directors nominates individuals who have demonstrated a lifelong commitment to community health and equity.
