Ngozi N. Moses, MSc, is the Executive Director of Brooklyn Perinatal Network Inc. (BPN) established in 1988 from a community task force dedicated to enabling collective efforts to address the inequities driving the health disparities in maternal child health outcomes. Ngozi is is currently providing leadership for a community-driven partnership initiative that included a coalition of CBOs with local health system champions, Pathways Community HUB Institute (PCHI), OH , and Health Systems Management (HMA) a national health consultants firm, teamed up specifically to introduce the Pathways HUB program model to NY via a demonstration project in Brooklyn for addressing social determinants of health.
